Number One Cause of Accidents

The number one cause of accidents is .... ?

Several years ago there was an accident involving a heavy guy who lost control on a 300-foot rappel using an ATC on a single strand of 8mm rope. The ensuing internet debate included some people insisting the accident was caused by the use of an ATC; the guy would have had more control with a different device. Others insisted it was caused by the use of an 8mm rope; skinny ropes should be outlawed. Others insisted it was caused by the use of single rope technique; the guy would have had more friction and control on a double rope. And others insisted the accident was caused because the guy wasn’t using an autoblock; self-belays should be used by every rappeller on every rappel.

In my mind all of those arguments are off-target, but when considered together point to what I believe is the number one cause of accidents.
